Description

Facilitate the launch of 4 innovation demonstrator sites in 2018/19 to scale innovative models of care within their local areas through effective commissioning. In conjunction, will launch an Innovation Network and Best Practice Exchange to capture and spread the emerging learning from the innovation demonstrator sites, enabling commissioners across the country to transform their approaches to commissioning care services, shifting from traditional models of care towards new innovations that promote independence.
