Description

Cambridgeshire County Council requires the design and delivery of evaluation to support the effective delivery of falls prevention services. The evaluation will provide the insight necessary to shape and inform the design and delivery of falls prevention services/pathway to improve the uptake of falls prevention interventions and contribute to a reduction in falls and falls risk in older adults living in Cambridgeshire. The Provider will be expected to undertake the evaluation on three distinct projects:
Project 1: A patient perspective of the local community falls prevention pathway
Project 2: A General Practitioner perspective of the GP Practice/PCN role in falls prevention and the local community falls prevention pathway
Project 3: An older adult perspective towards engaging in strength and balance exercise in later life.
The output is expected to be:
Project 1: An in-depth understanding of the patient experience of services
Project 2: A clear understanding of the issues faced by GPs/Primary Care in managing patients who have fallen as part of the multi-faceted falls prevention pathway and what they perceive as the opportunities for improvement.
Project 3: Behavioural insight into the low uptake and high attrition from the various strength and balance programmes to enable stakeholders to create strategies to improve participation and prevent, delay and reduce falls and frailty.
