Description
First for Wellbeing (FfW) is a Community Interest Company that has been established as a joint partnership between Northamptonshire County Council (NCC), Northamptonshire Healthcare NHS Foundation Trust (NHFT) and the University of Northampton (UoN) to create a single organisation for the development and delivery of services that enhance the physical, emotional, social and economic wellbeing of the citizens of the county. First for Wellbeing "the Commissioner" wishes to invite Tender submissions for the county wide supply of Social Wellbeing Services for vulnerable adults.
The successful Provider will be a very forward thinking organisation, capable of leading and managing Delivery Partners and taking forward our vision for social wellbeing using the principles of co-design and co-production to bring this exciting specification to life. They will be required to establish a single "one stop shop" holistic model of delivery where individuals are only required to "tell their story" once using First for Wellbeing's bespoke online wellbeing assessment tool - OCTIGO. The Provider will be required to work with the Commissioner maximising opportunities to bid for, and secure other sources of funding and developing innovative ways of engaging others; building social capital and sustainability, generating new income streams and improving outcomes for the population of Northamptonshire.
The Provider will develop and provide a seamless and equitable countywide service which enables people with a social wellbeing need to live safely and with dignity in their own homes and to continue living independently for as long as they wish to and / or to engage in meaningful occupation for as long as they are able. The target population are vulnerable adults, including those who are homeless or at risk of homelessness, those with learning or physical disabilities, those who are socially isolated, those at risk of losing their ability to live independently and those who need support to manage their finances.
First for Wellbeing are developing innovative approaches to assessment of individual need, recording individual engagement with services and pathways through service provision and evidencing the impact that our services have using their assessment tool OCTIGO. The Provider shall be required to work closely with Commissioners to support development of OCTIGO and associated systems, develop our shared understanding of its application and potential, and roll out its implementation across the Delivery Network.
The service will be required from April 2017 and TUPE will apply. This contract is for three years, with an option to extend for up to two years, subject to annual review. The maximum possible duration of the Contract, should the extension be taken up is five years.
