Description
East and North Herts CCG are looking to commission a structured education programme for newly diagnosed patients with type 2 diabetes (and their carers) to ensure they understand the condition and how they can self-manage their condition.
Diabetes education can help improve glycaemic control, self-care and emotional well-being and should be offered to every person and/or their carer at and around the time of diagnosis.
The programme needs to be flexible to meet the needs of individuals. As such the programme should provide both face-to-face group sessions and an online provision.
The Contract is for 3years, with an option to extend for a further 2 years.
