Description

Doncaster Council and Doncaster Clinical Commissioning Group (CCG) wish to commission a Social Prescribing Service in Doncaster. Social Prescribing is a means of enabling primary care services to refer patients who have one or more long term conditions with social, emotional or practical needs and support them to self-manage their condition by helping to develop/enhance essential life skills such as problem solving, setting goals, accepting change, finding coping strategies, managing relationships through good communication, and finding quality of life in difficult and challenging circumstances.
