Description
Aims and objectives of service
To provide an evidence based risk assessment and home exercise programme as part of a wider falls prevention strategy which engages with the community and increases capability and capacity in the community through the voluntary sector to sustain the interventions and promote falls risk awareness
To work as part of an integrated system linking existing services
Objectives
Signposting to the service via the Slough Services guide, general practices, the voluntary sector using a range of promotional materials e.g. website, leaflets, promotional adverts in the Citizen and demonstrations in community settings
Promotion of the service throughout falls prevention awareness month (1st-31st July)
Individual and group falls prevention exercise sessions lasting for 12 weeks to reach at least 300 people at risk of falling within 6 months
o Group and home based exercises to be based on the OTAGO programme
o Development of an integrated pathway linking the general practices, Dr Jha's service and exercise groups in the community
o Use of clear criteria from the medicines management team to facilitate medication reviews through local pharmacies
o Sit to stand blood pressure and mobile blood pressure monitoring
o Rapid referral for cataracts in those with bilateral cataracts via referral to the GP
o Correct use of safety shoes
o Bi monthly status reports for the Better Care Fund and individual patient outcomes to be reported to the persons GP at the end of the programme in terms of falls risk reduction and client satisfaction
3.2 Service description/pathway
o The service will provide an evidence based electronic timed 'get up and go' risk assessment and home exercise programme led by physiotherapists for a minimum of 300 clients in the first six months of operation and a further 300 by the end of the year
o A key feature of the service is access through the Falls Free 4 Life webpage which will be updated to include a Slough Borough Council and Slough CCG site; where GPs and A and E can encourage self referrals through a dedicated phone number or via email to info@fallsfree4life.co.uk
o The length of the intervention will vary according to the individuals risk and the evidence base
o The team will not work alone and will update the relevant GP/district nurse on the older persons status pre and post accessing the service.
o A sample machine will be purchased for Dr Jha's team allowing integration of services and a clear step up and step down protocol to be developed
o Working 9 - 5 pm weekday, and 9 - 1pm weekends the Falls Free 4 Life service for older people, would receive referrals from the GP and other practice nurses, as well as suitable cases from in-reach to the falls unit as advised by Dr Jha and from A and E through an automated extract to be established with Frimley Health.
