Description
The London Borough of Newham wishes to invite suitably qualified organisations to provide Dementia Community Memory Support services in the borough. The Council is looking to award a two year contract with a one year extension option. The Memory Support Service (comprising of Dementia Advisor, cafes, Singing for the Brain and young onset dementia activity) will augment the clinical pathway by providing a community based support service for people and their families living with dementia, both before and after diagnosis. This may help to delay their need for clinical support or hospital admissions. It will also help them to remain living independent in the community as long as possible. Contract value is upto £ 60K per annum. The contract is for 2 years with an option to extend for 12 months on council sole discretion.
