Description

Doncaster CCG is commissioning a service to provide comprehensive assessment and treatment, and provide advice and knowledge for wound care management. This service will enable non-housebound patients with wounds that require more specialist care, to achieve improved quality of life and independence.

To review and where appropriate treat patients referred by Tier 1 service or discharged by Tier 3 Wound Care services by providing:

• Specialist advice and service provision;
• High quality and safe-care using evidence-based interventions;
• Good access to services and education;
• Responsive patient-centred care. This will involve timely appointments in the neighbourhood where the patient lives (unless the patient chooses another neighbourhood)
