Description

Derbyshire GP practices to manage and oversee the delivery of The NHS Health Check
Programme for a period of 2 years from 1st April 2023 for a period of two years up to 31
March 2025.
The NHS Health Check programme aims to prevent heart disease, stroke, type 2 diabetes,
kidney disease, and raise awareness of dementia and alcohol harm both across the
population and within high risk and vulnerable groups within Derbyshire. In April 2013 the
NHS Health Check became a statutory public health service in England.
Local authorities are responsible for making provision to offer an NHS Health Check to
eligible individuals aged 40-74 years once every five years as set out in regulations 4 and 5 of
the Local Authorities (Public Health Functions and Entry to Premises by Local Healthwatch
Representatives) Regulations 2013, S.I. 2013/351.
