Description

The City of London are inviting quotations to deliver a Rapid Response, short term
intervention, home care service.

The service will be on a spot purchase arrangement intended to commence on 1st April 2022
for a period of 1 year, with the option to extend by one further year (1 year + 1 year). The
service will be on a spot purchase arrangement to facilitate rapid hospital discharge and/or
prevent admission for an initial 72 hours (in some circumstances this may extended for a
further 72 hours). It is anticipated that the value of this service will be approximately
£49,000 per year.

Providers are invited to submit their quotes for the Rapid Response, short term
intervention, Homecare service by responding to set questions and a pricing schedule to
propose a delivery model for a service that will:

- Commence on 1st April 2022 for a period of one year with the option to extend
by one further year at the City's discretion (1+1).

- A rapid response model that means that a response will be given within 2 hours
of the request being made and that a service will be provided to the person
within 3 hours of the order request being received.

- Deliver Home Care services, as required across 24 hours a day (if required) and
as agreed with City Adult Social Care.

- Have a single point of contact for all referring organisations to the Service
available from 8am to 9pm.

- Explain how you will verify how the activity delivered will help services users to
achieve their individual outcomes.

- Have a payment structure where charges will be made only for the activity
delivered and a costed hourly rate(s) in the pricing schedule for that activity.
