Description

Alzheimer's Society will deliver this project across South Gloucestershire and include the following services:

Dementia Support Service (30 hours) - working with Sirona, to provide and deliver a countywide service. The Alzheimer's Society lead on Clusters 4-6, whilst Sirona concentrate on Clusters 1-3. Working with Sirona, to provide information and sign-posting cover across the area, when needed.

The Dementia Support Worker service will be available between the hours of 9am - 5pm, Monday - Friday. There is also flexibility to offer out-of-hours support, to allow for the differing needs of service users. The Dementia Support Worker provides one-to-one support in people's own homes, or at an agreed venue, which can be easily accessed.

To provide telephone based information, sign-posting and support service, between 09:00 - 14:00 on weekdays, as a service to people with dementia, carers, family and friends, as well as professionals in Adult Services, Health and other community support services. Sign-posting across South Glos.

To support 2 Memory Cafes (Winterbourne & Hanham) and 2 Singing for the Brain® (SftB) services (Downend and Staple Hill), based in community venues, to ensure they can be accessed by the widest number of people. We will support service users to access these services by:

• Sign-posting service users to local community transport options.
• Encouraging networking through per support e.g. lift sharing
between service users.
• Continually review the locations of our services, to ensure they
meet the needs of the local community.
• Providing support with transport costs (for service users
attending Winterbourne Memory Café, who travel a substantial
distance)
