Description

NHS Calderdale CCG is seeking to commission a managed telehealth service for the lifetime of 44 patients, from 1 April 2021.

The supply, installation and maintenance of a digital health solution to be able to monitor/test the vital signs (blood pressure, oxygen saturations, temperature) of people with long-term conditions living in their own homes.

The solution to have the options of both Bluetooth and manual input, and readings to be able to be securely accessed remotely by authorised clinicians. The device should include a tablet with peripherals or a portal and peripherals. An automated system at a pre-agreed time, will notify the individual daily when they are due to take their reading.

The individual is asked questions around their general health and well-being and will record their reading.

Potential Providers to propose how much it would cost to provide the service to 44 patients, for their lifetime, based on a cost per patient model, therefore, as the patient numbers reduce, the contract value would reduce.

Confirmation of your capacity and capability to be able to mobilise the service to commence in April 2021.
Proposed service model that links to the requirements set out in this notice.
Propose how much it would cost to provide the service to 44 patients, for their lifetime, based on a cost per patient model, therefore, as the patient numbers reduce, the contract value would reduce.

This does not commit the CCG to undertake the procurement of the managed telehealth service. The CCG has the right to withdraw this notice.
