Description

NHS Corby and NHS Nene CCG's are looking to commission a service aimed at delivering clinically effective, appropriate, convenient and easy to access care closer to patients.
The aim is to commission a high quality, evidence-based, effective, equitable and accessible service for adults with lymphoedema, regardless of aetiology, that ensures improved, positive and sustainable patient outcomes in relation to clinical needs, quality of life, health and well-being. The CCGs' preference is to have a model of management founded on prevention, early diagnosis and intervention, and timely management of lymphoedema. The service should:

• Ensure early recognition, awareness and knowledge of lymphoedema, its prevention and management amongst all health care staff and professionals, and relevant patient groups.
• Encourage education, training and professional development for health care staff in relation to lymphoedema.
• Maximise supported self-management among those patients at risk of, and living with lymphoedema.

Duration of Contract
The contract will be for 3 years commencing as soon as possible with an option to extend for a further year.
