Description

Hammersmith & Fulham Clinical Commissioning Group, on behalf of the CWHHE Collaboration of Clinical Commissioning Groups (CCGs) is seeking expressions of interest from sufficiently experienced organisations in order to understand market ability and appetite to provide a 24/7, 365 day per year Telemedicine-clinical support function to support care homes across Central London, West London, Hammersmith and Fulham, Hounslow and Ealing Clinical Commissioning Group areas. This facility will be introduced to approximately 70 residential and nursing homes across the geography covered by these CCGs, with a phased implementation over the three years of the programme subject to successful implementation and performance of the preceding phases. Providers expressing an interest should have previous or existing experience of delivering this type of service, and be able to evidence the capacity and workforce in place to commence provision quickly, providing immediate advice and reassurance to both clinical and non-clinical staff and if necessary, to plan for local onward referral. If considered successful, the service will extend to patients and carers living outside of nursing and residential homes in further phases. Therefore, the CCGs will work with the provider over the course of the contract to seek to increase the breadth and value of the contract to improve care for people across North West London as part of the wider Last Phase of Life Programme.

The CCG anticipates awarding a 3 year contract with the option to extend for a further 2 years at their discretion. The anticipated annual contract value is £400,000 - £7,900,000 (subject to expansion of the programme and additional investment being agreed by the CCG). Should the CCG choose to undertake a formal procurement process, it is anticipated that a Contract Notice will be published during late January 2016, working towards full service commencement during April 2017. It is expected that the programme will be part of the End of Life Care Incubator, which offers support from Social Finance and a dedicated EOLC Incubator Advisory Board, opportunities for sharing learning with other similar UK projects, and access to funding from the Care and Wellbeing Fund, a social investment fund that supports community-based models of health and social care. Responses to this invitation may also inform how such a process may need to be designed.
