Description
The Council is proposing to award a 1-year contract with the option to extend for a further year, subject to satisfactory and budget availability commencing 6th January 2020 to a specialist provider who has relevant experience and a track record of operating a Trusted Assessor project/programme, ideally in the West Midlands area, as the service agreement will cover care homes in Birmingham, Sandwell, Walsall, Wolverhampton, Coventry and Solihull. The care home may also be outside of the city if it is a package re-start.
The successful provider will primarily work within City Hospital and Enhanced Assessment Bed (EAB) providers in Birmingham to facilitate approximately 1200 discharges* from hospitals and EAB providers to care homes per annum. This will be either a discharge to a short-term EAB; a package re-start to a long-term bed, or a new placement in a care home. The successful provider will also act as an independent and unbiased assessor who will support the Council, care homes, home care providers and Adult Social Care to provide a high-quality service to patients.
This service provision will ensure timely assessments are completed for citizens who require residential and/or nursing provision following their hospital or EAB admission thus ensuring a safe and timely discharge from hospital and EAB providers.
