Description
NHS Croydon Clinical Commissioning Group is INVITING PROPOSALS for the provision of Intermediate Care Bed Service for Croydon residents for the provision of a range of 'step- up' and 'step down' beds through joint working with social Services to ensure that Service User's physical, mental and social wellbeing needs are met and avoid acute hospital admission where it is appropriate to do so.
The Croydon vision for the Intermediate Care Bed service will support:
• The delivery of responsive rehabilitation and reablement services to people who would otherwise face unnecessarily prolonged hospital stays or inappropriate admission in acute inpatient care or long term residential care
• Professional working with a single assessment framework, single professional records, shared protocols and shared care with the aim of care planning documented and agreed with the service user or carer.
It is intended that the new service will commence from 31 January 2018.
