It is anticipated that the contracts shall run from 1st November 2018 for a period of 4 years and 11 months till 30th September 2023.

Lot 1 - Kirkley Mill Surgery - Maximum price per patient is £140.00. Based on the current weighted patient list size 6526.77 the estimated maximum contract value for the Service will be £4,492,594 over the lifetime of the contract.

Lot 2 - Nelson Medical Practice - Maximum price per patient is £140.00. Based on the current weighted patient list size 6350.23 the maximum contract value for the Service will be £4,371,074 over the lifetime of the contract.

Kirkley Mill Health Centre is open 5 days per week, 8:00 am - 6:30 pm. It is located on Clifton Road in Lowestoft, The Local Plan for New Waveney for the period 2014 - 2036, currently subject to consultation, identifies significant house building plans (9019 homes) in the area of which 5881 are planned to be built in Lowestoft.

The latest CQC inspection was carried out on 17 January 2018 following an inspection in 2017 which placed the practice in special measures. There was insufficient evidence to rate the practice in January 2018 due to a change in provider; the practice is rated as inadequate overall, inadequate for providing safe, effective and well led services and requires improvement for providing caring and responsive services and continues to be in special measures. The weighted patient list size is 6527 and actual list size is 6409 which is anticipated to grow in light of the housing and population growth.

Nelson Medical Centre is open 5 days per week, 8.00 am - 6.30 pm with early morning clinics available on Mondays and Fridays (7.30 - 8.00 am). It is located on Pasteur Road near the centre of Great Yarmouth. The latest CQC inspection carried out in July 2017 rated the practice as Good overall. The weighted patient list size is 6350 and actual list size is 6463 which is anticipated to grow in light of the housing and population growth.
