Pilot of a triple-faceted approach to outreach care to a) bridge the gap between young people's and adult substance misuse services to minimise the disengagement of young people from services as they reach 18, b) work with patients before and during rehabilitation to significantly reduce the risk of disengagement and the subsequent representations at services and increase the number of successful treatment completions and c) family and carer facilitator.
