Description
The service contract with West London CCG (WLCCG), now NWL CCG, is for funding the At Home Respite for carers of people live with Dementia. The service is run by Age UK K&C. Key elements of the service include:
• Offering a break for carers giving the cared for person a safe and supportive environment
• Supporting carers by enabling them to have a respite break from caring, so as to continue in their informal caring role
• Providing a safe and welcoming atmosphere for emotional support, social interaction and information
• Involvement of volunteers and health care professionals

• Improving access to healthcare,
• Working in partnership with both primary care and Hospitals to improve access to the range of pathways for ill health prevention,
• Health promotion and community treatment options.
• When a patient requires hospitalisation the projects support efficient and effective safe discharge protocols that ensure smooth transition back in to the community and reducing the likelihood of readmission.
• Ensuring there is a strong VSC offer strengthens our local community
It is anticipated that this not only improves health outcomes for this vulnerable group but may also reduce costs incurred by avoiding extended hospital stays and improving patient flows.
