Description

The individual is currently residing in a specialist placement (KPMS - Wolverhampton) funded fully by Children's services due to his LAC status. The placing authority was Staffordshire and the individual's care is managed by Newcastle District.

As the individual has turned 18 Adult Services have been requested to complete an Assessment under the Care Act 2014 and take over the funding of this placement.

Assessment completed on 08.12.2021, needs under the Care Act were established and assessment confirmed that the current placement was necessary and appropriate for the individual's ongoing care and support needs.
