Description

**PLEASE NOTE: This tender is subject to a pause in the process and the submission date is no longer Friday 18th August 2017 at 12pm. This pause is to allow the CCG to gather some further information for the tender. This pause is going to be effective immediately and will remain in place until Friday 1st September 2017. Relevant further information will be published on the 1st September and the new tender submission date will be advised at this time.

NHS Halton CCG has a requirement for a Community COPD Service. Operating across Halton, the Community COPD Service will provide home (including residential & nursing homes) and community based care. The service must be able to deliver:
• Patient centred care which allows patients with COPD to be more independent, taking more responsibility for their own care and quality of life.
• Seamless care and support to patients and their carers through close working arrangements with social care and 3rd sector providers
• A 'shift of care' into the community, working primarily with general practice as part of a multi-disciplinary team designed around, and for, the patient and carer
• Referrals of appropriate patients to the Community Pulmonary Rehabilitation Service

The Community COPD Service will also be required to make a significant contribution to NHS Halton CCG's plans to reduce non-elective admissions, to effectively manage prescribing costs and to improve the management of patients with COPD in primary care.

The contract will be awarded for an initial 5-year period with the potential to extend for a further 1 x 2-year period. The annual value of the contract is set at £750,000.

This procurement is for Social and other specific services which are Light Touch Regime services for the purpose of the Public Contracts Regulations 2015 as specified in Schedule 3 of the Regulations ('Regulations') http://www.legislation.gov.uk/uksi/2015/102/schedule/3/made
Accordingly, the Contracting Authority is only bound by those parts of the Regulations detailed in Chapter 3 Particular Procurement Regimes Section 7 http://www.legislation.gov.uk/uksi/2015/102/part/2/chapter/3/made
The Contracting Authority is not voluntarily following any other part of the Regulations.
The procedure which the Contracting Authority is following is set out in the procurement documents.
As the CCG is a relevant body for the purpose of the National Health Service (Procurement, Patient Choice and Competition) (No 2) Regulations 2013 these Regulations also apply to this procurement.

Right to Cancel: The Contracting Authority reserves the right to discontinue the procurement process at any time, which shall include the right not to award a contract or contracts, and does not bind itself to accept the lowest tender, or any tender received, and reserves the right to award a contract in part, or to call for new tenders should it consider this necessary.
