Description
The Know Diabetes Service (KDS) is a NWL digital health platform with targeted and tailored mass-communication functionality, directly catering to the needs of patients, carers and clinicians, powered by a patient database of over 296,700 people.
It hosts a range of content for self-led unstructured education, access to patient record access and provides access to REWIND and NDPP with significant patient outcomes. It is a first generation diabetes population health management tool that is delivered by the NWL CCG Diabetes Transformation Programme (DPT). The aim is to help more people living with diabetes achieve their three treatment targets and clinicians to support delivery of the nine key care processes.
