Description

The requirement is for a single supplier negotiation with Derwentside GP Federation (which acts as the lead for the six GP federations in the county) to extend the contract for the Cardiovascular Disease (CvD) Prevention Programme incorporating the NHS Health Check programme from 1 April 2023 to 30 September 2023.

NHS Health Checks is one of the five mandated public health functions in the Health and Social Care Act 2012 whilst the CvD Prevention Programme targets population groups with unrecognised risk factors for CVD that are not being identified through the Health Check programme
