Description
The Brokerage Service is a bespoke intervention to facilitate prompt hospital discharge where an individual requires long term residential / nursing care. Outcomes link to integration work and Durham County Council Adult and Health Services (DCC AHS) / NHS priorities on Discharge to Assess and reduction in Delayed Transfers of Care. The Brokerage Service will support Service Users who receive Local Authority funding, those who are funded through NHS Continuing Health Care and self-funders, who are not eligible for Local Authority funding but may need help and support to find an appropriate placement.
