Description
NHS Lincolnshire West Clinical Commissioning Group is the lead commissioner for NHS Continuing Healthcare for Lincolnshire. The 4 Lincolnshire CCG's - Lincolnshire East, Lincolnshire West, South Lincolnshire and South West Lincolnshire have a population of 758,622 dispersed population across 5,921 m2.
At this time the service requires support in delivering Continuing Healthcare (CHC) reviews on behalf of CHC Lincolnshire from appointment booking to the date of the next assessment review.
Requirement
The service requires a discrete clinical and administrative team working in the specific geographies of the 4 CCG across Lincolnshire, on assigned cases defined and prioritised by the CHC Department, managing the workload from initial appointment to next review ensuring all records are updated including recommendations regarding the package of care commissioned / provided.
The service to include;
• Meet with CHC team for Programme Briefing before commencing this work;
• Provide a trajectory for the completion of the reviews at the commencement of project
• Update the trajectory each week, reporting on number of reviews completed and any refresh of planned trajectory;
• Contact patients, their families, representatives and other professionals including members of the MDT/Local Authority, giving appropriate notice;
• book reviews;
• Conduct review, gathering essential information, confirming and measuring Material Change as per the National Framework (2018);
• Review care is appropriate and meeting patient need (Safe and Well and Deprivation of Liberties Safeguards considerations) and only funding is to be recommended to meet on going assessed needs appropriately and offers value for money;
• Complete all review documentation as agreed with the CHC team and send securely electronically within 7 calendar days of review; quality assurance audit; and the provision of reports and management information to the CHC Department;
• Participate in a weekly teleconference call with the Business and Clinical Manager to discuss issues/concerns;
• Provide a key point of contact for Business and Clinical Manager.
