Description

The aim is to support the NHS winter plan during 2017-18 and at a time of significant pressures in the health and care system so that patients can be referred to Social Prescribing services during a stay in hospital or a visit to A&E in order to support the discharge and reablement process and help people reintegrate back in to their local community once they return home. Indications suggest that Social Prescribing isn't as commonplace in secondary care as it is in primary care so this project will contribute to a growing body of knowledge
