Description

NHS Midlands and Lancashire Commissioning Support Unit Continuing Care Department on behalf of NHS Herefordshire Clinical Commissioning Group (CCG) is seeking to commission a 24-hour nursing care package for an individual with complex health needs. The individual has an acquired brain injury and requires support over a twenty four hour period at home, where the patient lives with their family in adapted accommodation. The above care must be provided for 7 days per week, 52 weeks per year. The patient's complex health care needs include: maintaining all health and personal care needs effectively on a daily basis and working closely with her existing multi-disciplinary team; medication administration via PEG (medication must be administered by a Registered Nurse and in accordance with NICE guidance); diet and fluid administration via PEG; pain relief as appropriate, careful monitoring of breathing and associated treatment, intervention to manage risk of epistaxis, support with continence. A full care specification will be provided when the tender opportunity is published. Qualified Nursing Staff will be required 24-hours a day to monitor general health, provide clinical oversight and provide training and support for the care team. One Care Support Worker (NVQ Level 3) will be required throughout the day and night (24hrs).

Commissioners are seeking to better understand which providers are in the market for the care package provision, as outlined above.
