Description

Community nursing is essential to delivering comprehensive healthcare, particularly in supporting patients with complex and long-term needs. However, there is an acute need for enhanced focus on end-of-life (EoL) and palliative care as due to populations age and the prevalence of chronic illnesses, community nursing teams must be equipped not only for general care but also for the specialised demands of palliative care. This shift is vital to ensure patients receive quality, personalised support in their preferred environment, typically their home or community setting.
The Aims of the project are to:
Analyse current practice and identify sustainable, high-impact models
identify and assess existing models of end of life and palliative care within integrated neighbourhood teams and the role of community nursing within the NEY region
understand best practise, identify gaps and recommend scalable patient centre care models.
