Description

The Council is looking to fund a service based on the MEAM (Make Every Adult Matter) approach. It is envisaged that this service will consist of an outreach service via a Pathway Worker which, will engage with clients who are referred by other agencies or who directly refer themselves. They will be street homeless or at risk of being street homeless.
The service will be funded from 1st April 2018 - 31st March 2021.
