Description

Expressions of interest - Prime Provider for services to people with a learning disability
Northamptonshire is embarking on an exciting and innovative transformation journey to secure sustainable improvements to the personal outcomes and life experiences of local people with a learning disability. Northamptonshire has a known population of 3,300 people with a learning disability, who have a range of abilities as well as support needs that include:

• Support to enter and maintain employment or voluntary work;
• Support to access social relationships as well as to maintain relationships;
• Support to access primary health care services;
• The use of packages of care following an assessment under the Community Care Act 1990 or the Care Act 2014 - packages can have varying degrees of intensity and be provided in a variety of locations;
• The use of secondary health services and learning disability speciality services;
• The use of packages of care funded by the Clinical Commissioning Groups, which may be of a continuing nature or short-term rehabilitation with a recovery-focussed nature; and
• The use of learning disability specific treatment and high acuity services provided under the Health Act 2006.

Northamptonshire County Council, NHS Nene Clinical Commissioning Group and NHS Corby Clinical Commissioning Group, alongside of NHS England Specialised Commissioning, form the Northamptonshire Transforming Care Partnership. This Partnership is charged with delivering the requirements of the national agenda of Transforming Care, as well as the local commitment to a wider learning disability service transformation, much of which is outlined in our local Transforming Care Plan. This plan describes our intention to provide a range of enabling and early intervention services, to move away from the use of institutional settings, to drive a further uptake of personal budgets across health and social care, and to support individuals to live ordinary, and sometimes extraordinary, lives.
