Description

NHS South, Central and West Commissioning Support Unit (SCW) seeks to commission an Integrated Lifestyle service that will provide a cost-effective, high quality and integrated lifestyle provision, that encompasses prevention and supports long-term behaviour change with respect to any aspect of healthy lifestyles.

SCW have proven behaviour change expertise and are seeking a partner that can deliver grass-roots support and work with existing local groups and communities through peer support, training and promotion of healthier lifestyles.

The service being commissioned will provide delivery of support to specific and targeted populations based on individual need, and utilise existing sources that already exist in the community, as well as harnessing the potential of the strong voluntary and community sector that are well placed to advise, support and promote health and wellbeing.

Key to the delivery of the programme is an ability to build on existing services that are reaching high risk groups, and to develop the 'making every contact count' approach.

The contract period will be a period of up to 3 years. The contract is expected to start in October 2018. The contract is estimated to be in the region of 150k per annum.
