Description

This is for the procurement of an app to support unpaid carers to support COVID recovery in the short to medium term, including supporting carers through potential rapid hospital discharge due to COVID, and the NHS Long Term Plan and CNO objectives in the medium to long term. As Primary Care currently register carers in their practice area, but do not always have the tools to support them the communication target will be PCNs and hospital discharge This is a business case to supply the Carers UK product (Jointly App linked with the Carers UK provision MyBackUp) systemwide (ICS) to support COVID recovery in the short to medium term, including supporting carers through potential rapid hospital discharge due to COVID, and the NHS Long Term Plan and CNO objectives in the medium to long term.
