Description
The main purpose of the service is to provide 24 hour accommodation, care, support and stimulation to adults for whom it is not appropriate, either in the short or longer term, to live in their own homes. The Service Provider should offer Service Users the opportunity to enhance their quality of life by providing a safe and effective service and ensuring people
have a positive experience of care.
This procurement seeks to enter into additional block purchasing agreements that will offer additional and fixed capacity of 9 beds for Older Persons nursing care. By securing such capacity the Purchaser will be in a stronger position to be able to manage increasing demand within existing financial and market-based constraints.
Transitional and Reablement Beds will provide various levels of support to Older Persons who require a period of further assessment to determine their longer-term needs with the focus being on returning home. This will be achieved in partnership between Purchaser and the Provider.
The service will provide early intervention, assessment, support and care planning for the Service User and will work with the individual and the Care Home staff to help achieve their identified goals and outcomes.
The procurement will be carried out by Lincolnshire County Council (LCC) on behalf of Lincolnshire Community Health Services (LCHS). Requirements include a block of Transitional and Reablement beds payable at the Older People High Dependency (HD) rate plus the current FNC nursing rate. This cost reflects the complexity and multi-purpose usage of the beds.
