· a sensory impairment
· a mental health need, including dementia/cognitive impairment.

· substance misuse It is not intended that the services that may be included in the Keeping Well Service are specific reablement services, but to support the achievement of the three key priority areas in our Living Well and Ageing Well Strategy for Adult Social Care in Sunderland we will expect all care and support services to be delivered with a reablement ethos.
