Description

The service will provide high quality supported and step down accommodation service which supports adult service users with high, moderate and low mental health needs to retain their independent living skills. The model emphasises on mental health recovery and rehabilitation and service users acquiring their own tenancy. Service users will be encouraged to live independently within the community, to access the same opportunities as their peers and to maximise life opportunities whilst maintaining links with the local community, family and friends.

The provider will need to provide evidence of sourcing the accommodation within Slough as stated below;-
a) An on-site supported accommodation service to approximate 8 - 10 self contained flats. The 10 flats will require on-site supported accommodation services available twenty-four (24) hours a day, seven (7) days per week including Bank Holidays with sleep-in/ waking night cover.
b) A approximate 4 - 6 unit building which will be step-down accommodation service for service users. Provider will deliver a floating step down support service to this 6 unit, one bedroom flats.

The provider will need to develop good partnership links within Slough, to ensure the move on process for a service user into an independent accommodation is smooth and easy. Provider will deliver a resettlement support service for a minimum period of three (3) months. The provider will deliver this support within the location the service user is relocated.

The main outcomes of this service is to;
a) Optimise physical, mental and social wellbeing and have a close working partnership with people to support their needs,
b) Instil hope, support ambition and balance risk to maximise outcome and independence,
c) Use an individualised, goal-based approach for service users to develop new skills and engage within education, training and employment opportunities.
d) Require early and ongoing assessment and identification of rehabilitation needs to support timely planning and interventions to improve outcomes and ensure seamless transition
e) Support self-management through education and information to maintain health and wellbeing to achieve maximum potential
f) Optimise independence to ensure service users are able to maintain their own independent tenancy.
