Description

Southend-on-Sea Borough Council is commissioning a six-month Pilot project for 4 bed spaces within an established residential care home in Southend. This is to provide a temporary period of assessment and enablement of people, either following a hospital admission or where there is a risk of hospital admission, who are unable to return directly to/remain at home due to requiring 24/7 assessment and enablement for a period of up to 14 days. This will be with a view to improving mental and physical well-being and ability, and increasing independence and confidence in order to maximise personal independence whilst enabling a holistic assessment of any on-going support needs to enable each person to return home once able, to continue their recovery in a non-residential setting. The term of the Pilot is for six (6) months, with the option to extend by up to three (3) months.
