Description
The London Borough of Croydon invites suitable expressions of interest and participation from potential providers for the 603/2017PH Procurement of its 'Just Live Well Mental Health Service'.
We intend to award this contract to one (1) strategic supplier for the delivery of the service. We anticipate that the contract will commence from September 2017 to the 31st March 2019 with the option to extend for an additional year.
Just Live Well - Mental Health' is a 12 month behaviour change programme offering lifestyle modules in smoking cessation, weight management, physical activity, alcohol, emotional health and well-being and health promotion for people with serious mental health illness.
The Provider is expected to use motivational interviewing (MI) to support their clients to develop awareness of their own motivation, and give them the opportunity to make their chosen behavioural change(s) - training will be available - and Providers will be expected to attend.
This service will deliver:
o An outcomes focused lifestyle behaviour change service addressing multiple behaviours (smoking, alcohol consumption, diet and nutrition, mental health and well-being, and physical activity) for people with serious mental illness.
o Integrated, holistic support to enable healthy lifestyle change that addresses barriers to making health choices.
o A single point of access that leads to a range of interventions, appropriate to the level of individual and community need.
o A targeted service offer that in its design will be accessible and suitable for target groups.
o A universal offer that makes good use of community assets and avoids duplications of internal and external services that support behaviour change through partnership work.
*As the Provider will be providing support to people with serious mental illness the Provider is expected to specialise in supporting and providing services to people with serious mental illness.*
