Description
The service provided meets a gap in Independent Advice and Support for Parent Carers of Adults with Learning Disabilities. This support covers, benefits, further education, housing, access to health for people with learning disabilities, concerns about services the person they care for is receiving and issues relating to carers own health and wellbeing. The service supports families whose son/daughter are transitioning from children's services into adult services ensuring continuity of support for parent carers who will be lifelong carers. The service identifies training options. The service also ensures that parents expertise is used in the ongoing development of local services.

• Working in partnership with both primary care and Hospitals to improve access to the range of pathways for ill health prevention,

• When a patient requires hospitalisation the projects support efficient and effective safe discharge protocols that ensure smooth transition back in to the community and reducing the likelihood of readmission.
• Ensuring there is a strong VSC offer strengthens our local community
It is anticipated that this not only improves health outcomes for this vulnerable group but may also reduce costs incurred by avoiding extended hospital stays and improving patient flows.
