Description

Any Qualified Provider (AQP), Adult Hearing Service for those aged 55 years and above with age related hearing loss, that are part of the registered population across the Calderdale, Huddersfield and North Kirklees CCGs footprint.

The Adult Hearing Service will offer a direct access service aimed at adults aged 55 and over experiencing difficulties with their hearing and communication and where a GP considers they may benefit from hearing assessment and care.

Any provider will be eligible to apply for this AQP contract if they can demonstrate compliance and competency against the specified criteria published within the tender documents.

The NHS supports the need to develop improved access to direct access services as part of the drive to reduce waiting times and improve choice options for Patients. Local services will improve access to care that is delivered at the right time and in the right place.

The overarching aims of the service are to:
• Provide increased patient choice and control as to where and when their treatment is delivered - providing on-going care closer to home
• Provide timely access to hearing assessment, fitting and follow-up
• Provide personalised care for all patients accessing the service
• Contribute to high levels of satisfaction from patients accessing the service
• Contribute to high levels of satisfaction from GPs referring into the service
• Support a reduction in social isolation and consequent mental ill health (i.e. depression and onset of dementia)
• Improve quality of life for patients, their families/carers and communication partners
• To continually work with stakeholders to develop and improve services
