Description

NHS Berkshire West CCG and Frimley CCG are seeking expressions of interest from providers who are able to set up and deliver a 2-year funded project to strengthen the wider pathway of support for autistic people aged 18+ across Berkshire - at any stage before or after diagnosis. Funding of up to £80,000 per year is potentially available for each of the 2 years (£160,000 in total).
The aim of the service is to enable autistic people to live healthy and fulfilling lives in the community and access the right support. Project aims include:

• Improve quality of life, independence and reduce health inequalities for autistic adults in Berkshire.
• Deliver autism pre- and post-diagnostic intervention support to autistic adults, carers and their families in partnership with health partners and local authorities, further developing best practice with national and local third sector organisations working with adults diagnosed with autism.
• Work in partnership with autistic people, their families and carers, health and social care commissioning organisations and VCSE colleagues to co-design and deliver accessible, person-centred, high-quality, multi-agency, practical, psycho-educational, and social support interventions for autistic adults.

It is expected this work will:

• Improve outcomes (e.g., mental, and physical health, quality of life, wellbeing, understanding of autism, autonomy, and independence) for autistic adults aged 18+
• Reduce loneliness and social isolation
• Help people to build resilience
• Contribute to the ambition to reduce unnecessary hospital admissions
• Provide early help to reduce the need for more intensive support services later on
