Description

Housing related support services for people with a mental health diagnosis within a variety of different properties and schemes across West Berkshire. This contract is initially for 3 years with the option to extend for an additional 2 years.

The provider will work with the service users to:
Maintain tenancies.
 Provide person-centred support, which takes full account of diverse needs and lifestyle choice.
 Promote Client's choice, well-being and safety.
 Promote people's independence by focusing on abilities and encouraging people to be as self-directing as possible.
 Manage the home environment.
 Manage finances, maximise income/benefits.
 Encourage access to employment/education.
 Encourage meaningful day-time activities, community access and integration.
 Engage with relevant professionals, i.e., Mental Health Team, Housing Options Officers, Employment agencies
 Establish and sustain effective social support.
 Reduce health inequalities by maximising access to income, learning and work.
 Advise on personal care.
 Provide high quality, cost effective and efficient services.
 Promote equality of opportunity and anti-discriminatory practice
