Description
Housing related support services for people with a mental health diagnosis within a variety of different properties and schemes across West Berkshire. This contract is initially for 3 years with the option to extend for an additional 2 years.
The provider will work with the service users to:
Maintain tenancies.
Provide person-centred support, which takes full account of diverse needs and lifestyle choice.
Promote Client's choice, well-being and safety.
Promote people's independence by focusing on abilities and encouraging people to be as self-directing as possible.
Manage the home environment.
Manage finances, maximise income/benefits.
Encourage access to employment/education.
Encourage meaningful day-time activities, community access and integration.
Engage with relevant professionals, i.e., Mental Health Team, Housing Options Officers, Employment agencies
Establish and sustain effective social support.
Reduce health inequalities by maximising access to income, learning and work.
Advise on personal care.
Provide high quality, cost effective and efficient services.
Promote equality of opportunity and anti-discriminatory practice
