Description
Stoke-on-Trent City Council in partnership with Stoke-on-Trent Clinical Commissioning Group (CCG) are seeking tenders from experienced organisations to co-ordinate a single integrated All Age Carer Support Service with a single point of access for adult and young carers from age 5 across Stoke-on-Trent.
The service will be managed by one single provider who may deliver the service in collaboration or through a series of sub-contracts or partnership arrangements. The provider will engage, collaborate and work with other organisations including community groups to deliver a broad range of interventions and access to local support options that meet the needs of carers and their families.
The provider will need to work closely with the local authority and health partners and will need to evidence collaborative working with a wide number of local organisations, voluntary sector and community groups to provide assessments, information, guidance and support for adult and young carers.
The service will support the legal obligations to carers in Stoke-on-Trent. This will include the undertaking of adult universal carers assessments and care and support planning in line with statutory guidance, alongside working with the local authority's young carers assessment team, adopting a 'whole family' approach to carer assessments, support and interventions, to help sustain and improve the carers health, wellbeing and independence both within and outside of their caring role.
