Description

Suffolk County Council is seeking a delivery partner for social prescribing services. The Council is part of an EU funded programme to develop local community-based services to improve the health and wellbeing of older populations affected by rural and social isolation and deprivation.

The programme will create a mobile social prescribing 'plus' scheme which will have the same components as a traditional social prescribing scheme but will be sited within a wider framework of early intervention and prevention services.

The provider will be expected to work closely with East Suffolk communities teams and SCC localities and partnerships teams to identify and develop a network of local service provision primarily within the non-statutory sector but linking to statutory bodies as appropriate.

The tender submission deadline date is 23 October 2019 at 12 noon.
