Description

NHS Newham CCG is working collaboratively as part of the North-East London Strategic Transformation Plan (STP) covering seven CCGs: Newham, Waltham Forest, City & Hackney, Tower Hamlets, Barking Havering & Redbridge. The vision for the STP is to create a simplified, streamlined urgent care system, which will ensure right care, right place, and first time access principles for patients in North East London. The key programme for the STP is the procurement of an Integrated Urgent Care Service (NHS 111 & CAS) that meets both local and national requirements.

The CCG is procuring an Urgent Treatment Centre with streaming and an integrated Rapid Response service. The population using the Urgent Treatment Centre will primarily come from within Newham, although there are increased attendances from the working age population who are temporarily living in and close to the Borough and the selected provider is required to set up a recharging process in line with National Guidance.
The selected provider will be expected to deliver the following core functions:
a. GP Streamer with local knowledge, based at the "front door" of the Emergency Department (ED) at Newham University Hospital (NUH) streaming ALL patients (excluding major care) that attend the A&E site.
b. A navigator service to support patients with medical assessment, advice, treatment, direction and self-care.
c. Provision of a minor illness and minor injury service between the hours of 8am to 11.pm (15 hours)
d. Provision of an additional minor illness service 11.00 pm to 2.00 a.m.(3 hours)
e. Receive referrals and bookings from integrated urgent care service (111 & the Clinical Assessment Service).
f. Book patients directly into GP practices and the 8-8 extended Primary Care Hub which is currently being piloted across Newham.
g. Assess patients for pathways to acute services, i.e. ambulatory care, Clinical Decision Unit pathways

In addition to the above, the service will include a Rapid Response team to support the clinical team in the Urgent Treatment Service by delivering the following additional functions:

a. Support patients at home which require short term acute medical care for a period of 0-3 days to avert unnecessary admissions and readmissions to the Urgent Treatment centre and Emergency Department.
b. Provision of urgent medical service (which responds to patients within two hours who are at risk of tipping into acute crisis) within a patients/service user's home or nursing home in Newham for any patient that goes into acute crisis.
c. Provide treatments and support to monitor the patient's condition.
d. Generate a reduction in hospital admissions for patients which require acute medical treatment.
e. Assist the Urgent Treatment Service with the post - discharge management of patients which require further acute medical support at their place of residence.
