Description
Redcar & Cleveland Borough Council and Middlesbrough Council, working in partnership with NHS Tees Valley Clinical Commissioning Group (CCG), are seeking organisations to help deliver the local agenda for carers across South Tees through the commissioning of a revised model of support. This approach will provide a broad offer for carers, increasing the identification of carers in community and broadening the interventions available to them. It will aim to increase the general awareness of carers not only in communities but across statutory and non-statutory providers, but importantly it will also empower carers to self-identify and direct themselves to a range of services available.
Providers can only bid for one Lot in Element 1, but they may bid for any amount of Lots in Element 2, however, we will expect all organisations commissioned, to collaborate in providing specialist holistic support.
