Description
Nottinghamshire County Council is seeking a suitably experienced provider to deliver a Hospital Discharge Service for Adults in the South of Nottinghamshire (excluding the City of Nottingham) under a framework agreement.
The Service will offer rapid response, short-term home based care to people who are ready to be discharged from hospital and have been initially assessed as requiring a home care package. It will deliver support while longer term services are being organised and will also help individuals maintain or regain their maximum achievable independence. The Service will be provided as part of a range of home based integrated care services.
The Provider must be appropriately registered with the Care Quality Commission (CQC).
The framework agreement will run for an initial period of 2 years. There is an option to extend for up to a further 2 years.
