Description

Community Physiotherapy and Community Pain and MSK Service
Surrey Heath CCG seeks to commission community based Physiotherapy and a Community Pain and MSK Service focused on the provision of high-quality care, designed around the needs of the local population.
The procurement consists of three Community Physiotherapy LOTS and one Community Pain and MSK LOT.
Services should be provided in locations that are convenient and accessible to local people.
The services should:
• Be designed around the specific needs of the population of Surrey Heath CCG, be high-quality, well-coordinated and patient-centred.
• Manage Pain and MSK referrals and proactively support the up-skilling of GP practices making referrals.
• Allow movement of patients seamlessly from one element to another
• Move to a biopsychosocial model of care which:
o Empower the population to self-care for their MSK issues and share responsibility of the management of their conditions, with a focus on education and lifestyle modification, as well as making effective use of local services.
o Focuses on mental wellbeing as well as physical diagnosis, assessment and treatment.
• Be supported by excellent clinical leadership and administrative processes to recruit and retain the highest possible calibre of multi-disciplinary teams
• Provide a seamless experience for patients with excellent communication at all stages in the journey and at transition points between services


The Contracts will be for a term of 5 years with a single option of extending the term for a maximum period of 2 years. The commencement date will be July 2017. The combined potential value of the Community Physiotherapy Service is approximately £5.0m (LOTS 1-3) and £4.4m for the Community Pain and MSK Service (LOT 4) over the seven-year term.
