Description

Overarching Aim:

The aim of the Service is to build the resilience of Carers (of people with Dementia) to maintain their caring role for as long as they choose and thereby reduce inappropriate admissions to hospital or premature admission to long term Residential Care.

Outcomes:

The Commissioner would want the Service to co-produce meaningful outcomes measures with the people accessing the Service. However, the Commissioner would anticipate outcomes to be demonstrated in the following broad areas:

Outcome 1 - Engage and involve people in all aspects of Service Delivery

Outcome 2 - People who have accessed the Service report that staff were caring, compassionate and understanding.

Outcome 3 - People accessing the Service show positive signs of increased resilience.

Desired Outcomes for Carers

• Reduce social isolation and the stress associated with caring.
• Improved access to local community support including mental health support services such as Talking Therapies.
• Improved knowledge and understanding of the care system.
• Increased resilience to reduce the risk of care breakdown and inappropriate hospital admission.

Desired outcomes for the person with Dementia

• Care needs continue to be met whilst their carer has a break.
• Increased access to social stimulation and local outings.

Outcome 4 - The workforce is resilient and supported through the provision of supervision and continued professional development.

Outcome 5 - The Service is flexible and responsive, adapting to the individual needs of the person in terms of their circumstances i.e. strengths, level of risk, culture, ethnicity, language, age and disability.

The Contract will be for a term of two years with the opportunity to extend for one year and a further one year subject to mutual agreement between the successful Bidder and the Authority. The maximum term of Contract is consequently four years.


The maximum budget available is £179,376 per annum
