Description

Following a review of existing separately commissioned, housing related support contracts which highlighted the crossover in services, Housing and Community Services (HCS) and Commissioning and Improvement People Services (C&I- PS) are undertaking a joint recommissioning approach. With agreed synergy about what is needed from future housing related support services the joined up approach aims to deliver cost effective services that continue to meet the needs of the people of Poole. The procurement incorporates 3 main pathways of provision. These are:
• An Assertive Outreach Support Pathway
• A Supported Accommodation Pathway (Adults)
• A Young Persons Pathway

Lot 1: (Only one Provider required)

Provision of a Housing First Support Service.
The Service offers support for individuals who have a history of entrenched rough sleeping, who may be difficult to successfully house or who refuse to engage with support and who may have multiple complex needs. The Service will provide intensive support with a focus on harm minimisation and engagement with relevant support agencies. The Service Model offers immediate secure long term housing without any requirement to show capacity for tenancy readiness. In order to facilitate support, six units of accommodation will be sourced by the Service Commissioner in line with the minimum requirements of the Service Specification.

Provision of an Outreach Support Service.
This Outreach Support Service will be aimed at supporting single people and couples to develop or maintain their independence within the community, thus preventing loss of their home or tenancy. The type and quality of support will enable Service Users to live as independently as possible in the least restrictive setting resolving experience of housing difficulty.
Lot 2: (Only one Provider required)

Provision of High Support Accommodation For Adults with Complex Needs
This provision is for a high level supported housing service. The Service will support adults with high and/or chaotic, complex support needs, who are presenting as Homeless or at risk of Homelessness.

The Service will also include an element of time limited Resettlement Support where Service Users move into settled and independent accommodation directly from the high support provision. The focus will be to ensure that Service Users understand their new tenancy requirements and living environment.

Provision of Transitional Accommodation For Adults with Complex Needs
This Service will support adults with low intensity support needs, who are transitioning from high level supported accommodation or require time limited and low intensity support to assist the Service User to develop skills and knowledge to move on to independent living.

The Service will also include an element of Resettlement Support which will be provided for a time limited period to all Service Users moving into settled and independent accommodation. The focus will be to ensure that Service Users understand their new tenancy requirements and living environment.
Lot 3: (Only one Provider required)

Provision of High Support Accommodation For Adults Experiencing Mental Ill Health
This provision is for a high level supported housing accommodation service. The Service will support adults who have a formally diagnosed mental health illness and may be subject to the Care Programme Approach and those who are in the process of being assessed/diagnosed by a Community Mental Health Team

The Service will include an element of time limited Resettlement Support where Service Users move into settled and independent accommodation directly from the high support provision. The focus will be to ensure that Service Users understand their new tenancy requirements and living environment.

Provision of Transitional Accommodation For Adults Experiencing Mental Ill Health
This provision is for transitional supported accommodation for Service Users who are transitioning from high level supported accommodation or require time limited and low intensity support to assist the Service User in managing settled accommodation.

The Service will include an element of Resettlement Support which will be provided for a time limited period to all Service Users moving into settled and independent accommodation. The focus will be to ensure that Service Users understand their new tenancy requirements and living environment.
Lot 4: (Only one provider required)

Provision of High Support Accommodation For Young People aged 16-25 years with Complex Needs
This high support accommodation scheme will be for young people aged 16 to 25 with complex support needs and/or challenging behaviours. The focus of the Service will be on those aged 18 to 25 however there may be occasions when a 16 or 17 year old has needs which would be well met within this accommodation offer.
Lot 5: (Only one provider required)

Provision of Training Supported Accommodation For Young People aged 16-25 years
This provision is for a Training Supported Accommodation Scheme for young people aged 16 to 25, with low intensity support needs, who are transitioning from high level supported accommodation or require time limited and low intensity support to assist the Service User in managing settled accommodation. The focus of the Service will be on those aged 18 to 25 however there may be occasions when a 16 or 17 year old has needs which would be well met within this accommodation offer.
The Borough of Poole has requested TUPE information from current providers of this service. It is for you to decide whether or not TUPE will apply.

We are providing the TUPE information received to date but the Borough will not and cannot be liable for the accuracy of this information and it is supplied as guidance only. If further information is supplied to us, we will forward this to you. The TUPE information will be available after expression of interest.
Applicable to all lots
• Accommodation is in place to service the minimum number of units or a commitment to have accommodation in place at the point of implementation.
• All staff shall have Enhanced Disclosure and Barring Service (DBS) clearance before commencing employment and DBS checks will be reviewed at least every 3 years.
• Maintenance of a data management system which is web based on a secure server and provides case management (with read only access for Commissioners) and the flexibility to respond to changes in monitoring data requests and produces quarterly/annual reports.
Applicable to Lots 2, 3, 4 and 5 only
• The building should adhere to the Housing Act 2004 Part 1, Housing Health and Safety Rating System (HHSRS).
• The Service Provider will work with the Service Commissioner and other relevant partner agencies in order to make reasonable adjustments to the accommodation to ensure it is accessible for Service Users, inclusive of those with physical disabilities.
• Smoke alarms in all Service Users rooms and communal areas.
• Access to a communal garden or other private outdoor space
