Description

Yorkshire and Humber Commissioning Support (YHCS), on behalf of NHS Hull Clinical Commissioning Group and Kingston upon Hull City Council, is seeking to commission community care equipment service and integrated wheelchair services through a restricted procurement process. The community care equipment service element is jointly commissioned by NHS Hull Clinical Commissioning Group and Kingston upon Hull City Council, and the integrated wheelchair service solely commissioned by NHS Hull Clinical Commissioning Group. The preferred Bidder will be required to demonstrate how they will deliver services in an environment of transforming health and social care provision as outlined in the NHS Five Year Forward View, the Care Act 2014, and Hull 2020 Programme.

The services aim to enhance and maintain the quality of life and independence of children, young people and adults with care and support needs, including those with temporary/permanent and/or life-limiting conditions. It will support the commissioners in the successful delivery of reablement and rehabilitation pathways as an alternative to secondary care, avoiding unnecessary hospital admissions and delayed discharges. The services will provide high quality, cost effective services to children and young people and adults in their place of residence, school, work and other appropriate environments as determined by the service user's identified needs.

The service is required to incorporate the following principles within the service delivery model:
• Right access - the service delivery model needs to be mobile and flexible to meet the varying service user needs
• Right timing - timely access to assessment, supply, maintenance and repair of care equipment, wheelchairs (and associated equipment)
• Right assessment (wheelchairs) - specialist wheelchair assessment and review for children, young people and adults with a long term or short term condition/illness
• Right provision of care equipment and wheelchairs - facilitating service user choice within defined eligibility criteria.

The provider will be required to deliver an outcome-based service to include:
• High levels of service-user satisfaction (including parents/carers)
• Logistics - storage, supply, and deliveries (timely, supply and collection)
• Provision of clinical specialist wheelchair assessment (and associated equipment)
• Effective and efficient supply of care equipment to service users, to support them and their parents/carers
• Repair and maintenance of wheelchairs and care equipment, including reconditioning and modification, where appropriate
• Robust business management systems and processes including stock control, waiting list management.
